Person-centered Planning: Support for the Transition Process and Developing Self-determination

Abstract
 
The successful transition of youth with disabilities from school to work and integration into the community is a topic that has received a great deal of attention from consumers, parents, and professionals, especially in the past decade. Many important studies and projects have addressed the issue of how to ensure that the school to work transition is both meaningful and successful. However, it is disappointing that the "outcomes of youths with disabilities exiting public schools have not improved significantly during the last decade". This paper looks at some of the literature on how the transition from school to work can be more effectively prepared and identifies a collaborative person-centered planning from the pre-school stage, which is based on the self-determination principle.
